In temporary incapacity (pay to client):

Claim Form(Fully completed, dated Signed and stamped)
Salary slip of employee;
Claimant photo ID certified
Medical Expenses;
Doctor Certificate;

In case of incapacity

Pay to employee

Claim Form (Fully completed, Dated Signed and stamped)
Salary slip of employee;
ID Photocopy of employee certified;
Mapa de Junta (% of incapacity) original or certified photocopy;

In case of Death

Pay tobeneficiary

Claim Form (Fully completed, dated Signed and stamped)
Salary slip of employee;
Claimant photo ID certified
Certified ID Photocopy of beneficiaries;
Original Death Certificate or Certified photocopy
